Del Oro Parents Club Safe & Sober Grad Nite - Golfland Sunsplash 2026 Parent/ Guardian Approval & Student Medical Release
My student/participant, has my permission to participate and attend Del Oro Parents Club Safe & Sober Grad Nite - Golfland Sunsplash 2026.
Medical Treatment/Emergencies - In the event that I, or another parent/guardian cannot be reached in an emergency, I give the Del Oro Parents Club Safe & Del Oro Safe and Sober Grad Nite Committee or their contractors, agents, employees or volunteers, and staff the authority to obtain immediate medical attention for Participant, regardless of age. I do hereby consent to all x-ray, examination, anesthetic, medical, surgical, or dental diagnosis or treatment and hospital care deemed necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under the supervision of the medical staff of the hospital or facility furnishing medical or dental services.
Allergies - I (we) hereby advise that the student/participant has had the following allergies, medicine or food reactions or unusual physical condition, which should be made known to a treating physician or which could limit participation. I will email allergies, medicine, and food reaction specific instructions to delorogradnite@gmail.com.
I understand and acknowledge that neither Placer Union High School District, Del Oro High School, Del Oro Parents Club, nor the Del Oro Parents Club Safe and Del Oro Safe and Sober Grad Nite Committee or their contractors, agents, employees or volunteers insure Participant’s or any other party’s behavior, actions or participation in the Grad Nite event and that none of the above assume any liability whatsoever for personal injuries or property damages to Participant or to third persons arising out of the transportation to or from Disneyland or participation in the Grad Nite event. Participant and Parent/Legal Guardian expressly waives any claim of personal injury or property damage arising from participation in the Grad Nite event.

The undersigned parent(s) or guardian(s) assumes all risks in connection with the student’s participation in all Del Oro High School Parents Club sponsored activities. I (we) hereby release and discharge the Del Oro High School Parents Club and all Del Oro High School Parents Club Booster officers, employees, agents, chaperones, and volunteers from all liability, claims or demands for any damage, loss or injury to the student, the student’s property, or parent’s property in connection with participation in these activities.
I (we) do hereby certify that to the best of (our) knowledge and belief that said participant is in good health. In case of illness or accident, permission is granted for emergency treatment to be administered. It is further understood and agreed that the undersigned will assume full responsibility for any such action, including payment of costs.
Waiver of Claims - I understand that California Education Code Section 35330(d) provides that all persons participating in an excursion shall be deemed to have waived all claims for injury, illness or death occurring during or by reason of an excursion against Placer Union High School District, Del Oro High School, Del Oro Parents Club, the Del Oro Parents Club Safe and Sober Grad Nite Committee or their contractors, agents, employees, or volunteers.
Release, Waiver and Discharge - I hereby release, waive and discharge Placer Union High School District, Del Oro High School, Del Oro Parents Club, the Del Oro Parents Club Safe and Sober Grad Nite Committee or their contractors, agents, employees or volunteers, its Board of Directors, officers, employees, and agents from all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death or loss or damage to personal property, occurring during or by reason of participation in Sober Grad Nite, including, but not limited to travel to and from Sober Grad Nite. I fully recognize and understand that if Participant is injured, dies or his/her property is damaged, Participant is waiving his/her rights (as well as the rights of his/her heirs, executors, administrators or assigns) to make a claim or file a lawsuit against Placer Union High School District, Del Oro High School, Del Oro Parents Club, the Del Oro Parents Club Safe and Sober Grad Nite Committee or their contractors, agents, employees or volunteers even if Placer Union High School District, Del Oro High School, the Del Oro Safe and Sober Grad Nite Committee or their contractors, agents, employees, chaperones or volunteers negligently or by some other act or omission causes the injury, death or damage.
Acknowledgement of Risks - I acknowledge and understand that the activities involved in this excursion may be dangerous and include risks which are inherent and cannot be reasonably avoided without changing the nature of the activity. I acknowledge that participation in the activities can cause personal injury, including emotional trauma and even death.

SUNSPLASH TERMS & AGREEMENTS

Del Oro Parents Club Safe and Sober Grad Nite 2026 - Golfland Sunsplash Waiver I understand the following...
• Del Oro Safe and Sober Grad Nite is a “Zero Tolerance” event. Any graduate, regardless of the age, choosing unacceptable behavior will have their parent called and will be picked up by the parent and/or counseled by local law enforcement.
• Only seniors from Del Oro Class of 2026 can attend. STUDENT ID IS REQUIRED AT THE TIME OF CHECK IN.
• Graduates must Check-in at the venue between 10:00 pm and 11:00 pm to be admitted. Gates will close at 11:00 pm. We will have a specified check-in area, where each student signs themselves in, bags are checked, and students may be breathalyzed. We will do our best to notify parents if a registered graduate does not check in.
• Graduates (regardless of age) will NOT be allowed out of the event boundaries until 3:00 am unless grad nite organizers have spoken directly to their parents. Volunteers will do their best to ensure that students do not exit or enter during the event but will not be held liable if that happens.
• Bags/Belongings will be searched and bags can be left at bag check. We will have volunteers at bag check to ensure the graduates items are secure for the duration of the event. The graduates may access their bag with the help of a volunteer throughout the night. NO ALCOHOL, DRUGS, TOBACCO, WEAPONS, LIQUIDS OR OUTSIDE FOOD on the premises. All open containers will be confiscated; please leave perfumes, lotions, hand sanitizer, liquid lipstick, water bottles, unopened beverages etc. in your car.
• Photos may be taken by the volunteer team at the event and used in promotional materials and/or shared with the attendees.
• Payment and costs for Del Oro Safe & Sober Grad Nite at Golfland Sunsplash are nonrefundable.
• Finally, we strongly recommend that a student does NOT drive themselves to and from the event. We encourage parents to drop off and pick up their students to ensure their safety. Driving exhausted can be compared with driving drunk.
